Today, as the spread of Coronavirus (COVID-19) continues throughout communities, we want you to know that we are here to support you with access to, and support for, our products. This page provides answers to your COVID-19-related questions.

As product demand has evolved over the past several months, the majority of BD and Alaris infusion sets are no longer on manual inventory allocation. However, we will temporarily maintain a limited catalog of BD Alaris™ pump sets.

We continue to provide our distribution network and channel partners with regular updates on product availability to ensure deliverability through standard channels.

Your BD account management and clinical consultant teams are available to assist you with any questions regarding product status or clinical practice. For information on available SKUs and product substitute recommendations, please refer to the IV Set Formulary Catalog.

We understand that while distribution is suspended for the BD Alaris™ System, you may have an immediate medical need that requires a limited shipment. As a result, we have established a process by which existing customers with Alaris™ System devices may request limited shipment of the BD Alaris™ System software compatible devices upon certification of medical necessity. Due to increased COVID-19 demand, BD will limit approved medical necessity orders to an equitable allocation to ensure we effectively serve our existing customers as well as serve areas of the country with high census due to treatment of COVID-19 patients.

Due to the current COVID-19 crisis, our team has been receiving many questions from BD Alaris™ Infusion System and BD Alaris™ Pump Administration Set users. Below are clinical considerations and resources to address those concerns.

Resources

To ensure that your devices operate efficiently, use the recommended healthcare-grade cleaning products and correct cleaning and inspection processes defined in the applicable Alaris™ System User Manual. Several of the recommended cleaning products for the Alaris™ System have also been identified by the US Environmental Protection Agency (EPA) as products that meet their criteria for use against SARS-CoV-2, the cause of COVID-19. BD has ramped up production and continues to meet current demand for the vast majority of our products. However, we are seeing high demand in select Alaris™ System pumps and IV administration sets because of a prolonged flu season and the coronavirus pandemic. We are closely monitoring this dynamic situation and will keep customers informed on any meaningful changes to product availability as the situation evolves.

We have been receiving customer inquiries regarding extending pump set change intervals for the BD Alaris™ Pump Infusion Set. The BD Alaris™ Pump Set labeling includes the following related to set change intervals: “The set should be changed according to facility protocol or in accordance with currently recognized guidelines for IV therapy.” BD Alaris™ functional performance testing for rate accuracy, occlusion detection and air in line supports the use of the BD Alaris™ Pump Infusion Set for up to 96 hours (excluding blood or blood products). The information here provides well-accepted guidelines on extended set change policies to support conservation efforts in the event of low inventories or backorder situations for the BD Alaris™ Pump Module administration set.

The US Environmental Protection Agency (EPA) published a list of products that meet their criteria for use against SARS-CoV-2, the cause of COVID-19. The following products from the EPA list are also approved cleaners listed in the Cleaning Product Guidelines for the Alaris™ System:

  • 10% bleach (sodium hypochlorite, one-part bleach to nine parts water)
  • Clorox Healthcare® Bleach Germicidal Wipes
  • PDI® Super Sani-Cloth® germicidal disposable wipes (purple-top)
  • Wexford Labs Wex-Cide 128 concentrate germicidal detergent

BD Pyxis™ MedStation™ Education Videos Quick Reference Guide
The education needs of our customers are shifting to meet the steady increase in temporary staffing models, making it difficult to utilize a Learning Management System. To accommodate these needs, BD now provides those who desire a different option outside of an LMS direct access to the BD Pyxis™ MedStation™ System Training Videos. The purpose of this quick reference guide is to provide the direct links to BD Pyxis™ MedStation™ education videos as resources for customers. The education videos are for the BD Pyxis™ MedStation ES and for the BD Pyxis™ MedStation™ 4000 devices. The BD Pyxis™ MedStation™ ES can be used for versions 1.3,1.4,1.5, and 1.6. QR codes are also provided so that learners can scan the QR code directly from their mobile device and then watch the videos directly from their mobile device.

In response to the COVID-19 situation that Health Systems are facing, BD remains committed to provide a consistent approach to delivering field services and support. To ensure the safety of BD and customer staff, the BD Global Customer Services Team will work closely with our customers to determine the criticality of each service activity, timing, and the requirements to be on-site. Our team has expanded its capability to utilize remote options as much as feasible with collaboration and cooperation from our customers.

BD Pyxis™ FAQs

We recommend you clean equipment often. Surfaces with direct exposure to patients (e.g., procedural area, isolation area) should be cleaned between patients.1

  • We have a summary list of cleaning solutions that have been tested to be compatible with BD Pyxis™ equipment.
  • BD Pyxis™ Cleaning Guidelines are available here.
  • The current CDC guidance states that “routine cleaning and disinfection procedures (e.g., using cleaners and water to pre-clean surfaces prior to applying an EPA-registered, hospital-grade disinfectant to frequently touched surfaces or objects for appropriate contact times as indicated on the product’s label) are appropriate for SARS-CoV-2 in healthcare settings, including those patient-care areas in which aerosol-generating procedures are performed.”2
  • The Environmental Protection Agency List “N” includes all commercial products that meet EPA’s criteria for use against COVID-19.2
  • Regarding the issue of cleanability of BD Pyxis™ matrix bins and cubies, we also invite Pyxis users to review the use of disposable bin liners (example available here). A 2005 study entitled “An Evaluation of Medication Drug Bins as a Potential Source of Nosocomial Pathogens” (supported by HealthCare Logistics Company) advocated that “regular cleaning of bins or use of bin liners should be considered as a strategy to reduce nosocomial infections” and that bin liners may be a “convenient, quick to use, and low cost in comparison to a bin cleaning program” (reference).

The Centers for Disease Control and Prevention has released guidelines:

A “clean hands” approach involving “wash in before” and “wash out after” is advised for accessing medications from dispensing devices.

Medications should be replenished with a “clean hands” approach beginning in central pharmacy, covered/protected during transport, and hand washing before touching BD Pyxis™ and the medications that will be loaded/stocked. In areas with high potential for exposure, it may also be advisable that pharmacy staff wear gloves (in addition to any other personal protective equipment that is deemed warranted) and discarding it before leaving that area (helping to prevent spread). Gloves should not be considered a substitute for proper hand hygiene.

With a significant proportion of inpatients being treated as COVID+ or presumptive positive, we are seeing hospitals leaving used medications in the patient room. This is especially the case for multi-use, bulk, or patient-specific items (e.g., creams, ointments, eye drops, inhalers, spacers) regardless of COVID+ status. We can appreciate the challenge (especially those setting up temporary isolation rooms) in meeting this requirement during these difficult times. The TJC website reminds us that their definition of secure does “not prescriptively require medications to be locked (unless required by law and regulation),” and their intent is that medications are “protected from unauthorized access, tampering, theft or diversion”3.

Unused sealed items (e.g., medications, supplies, RSI kits, code trays, etc.) simply may not be feasible to clean in a meaningful manner (e.g., alcohol sometimes causes the labels to bleed and become unreadable, vials have uneven surfaces and crevices). In today’s environment of medication shortages (expected to get worse), strategies to avoid throwing these items away are needed. Considering the recent evidence that suggests the coronavirus can remain viable and infectious in aerosols for hours and on surfaces up to days4 (van Doremalen, 2020), many are looking at alternative strategies like sequestering these items for a period of time. At NYU Langone, Arash Dabestani (Sr. Director of Pharmacy) recommends having a plan for the process of cleaning and replacing crash carts from isolation areas. “It’s best to have a dedicated space to process and quarantine returned meds and med trays from isolation areas for a few days5.” Others are sequestering items in rigid storage containers for up to ten days6. Because the duration of coronavirus viability varies by surface type and the initial viral load, duration recommendations are still not clearly defined5. We recommend the “SFAIRP” approach (so far as is reasonably practicable) and sequester drugs as long as feasibly possible during these difficult times.

BD Pyxis Supply is an inventory optimization system that helps to automate supply distribution across the enterprise (hospital or IDN) by providing reports from one system that aid in improving financial performance, clinical efficiency, patient safety and regulatory compliance; while providing a verifiable return on investment.

For customers that have BD Pyxis Supply in place, there are several options available for managing Personal Protective Equipment (PPE).

  • When PPE is needed in locations that have BD Pyxis Supply equipment –
    • Load the PPE into existing storage locations that have available space.
    • If additional space is needed, add additional auxiliary units to support additional of PPE.
  • When PPE management is needed in a new location – Add expansion units to existing system and consider the short term leasing options BD is making available to assist in the current situation.

1. For non-BD Pyxis Supply customers, the complete Supply infrastructure including server, client workstation and point of use devices would need to be implemented.

2. For current BD Pyxis MedStation customers, you could consider adding the PPE items to the system formulary and use your MedStation devices to manage access to PPE. This would be straightforward for control of inventory and traceability of use but may not drive replenishment through the ERP system without additional configuration at the CCE level.

When considering repurposing BD Pyxis™ Supply inventory management solutions from one location to another, it is recommended to review the system reporting to determine which devices have a lower level of utilization for products and prioritize those for potential redeployment. Repurposing a device that drives a high volume of utilization or manages an amount of inventory that is significant from a value perspective would not be recommended.

To reinforce appropriate infection prevention and control behaviors, SHEA and BD are pleased to announce the launch of the Prevention Course in HAI Knowledge and Control (Prevention CHKC). BD HealthSight™ FAQs
  • Although the Shortage Manager is primarily intended for managing actual drug shortages, you can create shortage cases for any medication in your formulary.
  • Medication use is typically evaluated based on an average usage in the past 120 days and will not reflect an acute change in usage volatility.

    • The Shortage Manager uses metrics based on the last seven days of data, permitting you to more accurately forecast inventory needs based on current average daily usage.
    • If there are medications with the similar therapeutic use, assigning them as Substitutes enables you to monitor the inventory days on-hand of the whole group of medications.
    • The shortage manager permits organization of shortage cases by priority, which makes it easier to locate and monitor the cases with highest priority.
  • To use this tool to monitor your usage of critical COVID-19 medications, create a shortage case for each medication. The case will permit you to see where that medication currently is stored, where it is (and is not) being used, days on hand, and estimated stock-out date.
  • When you experience a medication shortage at a facility, you may want to know how much of the medication the facility has, where it is located, and how fast the medication is being used. The Shortages page allows you to view and monitor pre- and post-shortage data.
  • Once you determine a medication that you want to monitor, you can create a Shortage case.
  • If you choose, you can adjust PAR levels from within the Shortage case or set them back to pre-shortage levels when the Shortage case is closed. The Shortages page provides two views:

    • Health System Shortages (a consolidated view of medication shortages across all BD HSIO enabled facilities within the Health System).
    • Facility Shortages (a view of medication shortages associated with the currently selected BD HSIO enabled facility).
  • If you are managing these drugs for an enterprise in multiple EMR systems, usage of the same medications across the enterprise may be viewed from a single point. A connection through BD HealthSight™ Data Manager with formulary mapping to the BD Library permits you to monitor drug usage with multiple medication identifiers (Med IDs) across your enterprise.

No, you will proactively manage PARs within the Shortage Manager. By default, the list of shortage medications is sorted by estimated stock-out date in chronological order. Furthermore, the estimated stock-out date is displayed in red when it is less than or equal to seven days from the current date for active cases.

If you want to see PAR recommendations from HealthSight™ Inventory Optimization (IO), it is just a few clicks away.

Whenever you increase inventory, there is a risk of items expiring before use. To avoid this risk, use the inventory snapshot feature of the Shortage Manager to capture pre-emergency PAR levels, and restore those PAR levels when usage returns to normal.

Preparing Patient Care Area devices for increased inventory amounts done by performing the following:

  • Determine the desired inventory level
  • Remove items from care area devices that are not being used to create room for the additional inventory
  • Where needed, replace pockets that are near capacity with larger pockets that can hold the desired amount of inventory, or place additional pockets in the freed-up space. Pocket type is on the Medication Details page.

During the pandemic, it is of utmost importance to ensure that needed medications are available, while reducing trips to Patient Care areas to avert potential exposure. This means that your inventory cost will temporarily increase. Use the inventory snapshot to reduce inventory when usage returns to normal to relieve that inventory.

View the inventory valuation on the Inventory page across the enterprise as well as by facility and export to a .CSV. Filter by type to view just selected mediations such as Shortages and Substitutes.

  • The Soon to Expire Items report looks forward in time to identify medications, in BD Pyxis™ MedStation ES devices, that are at risk of expiring in the near future.
  • The report also identifies devices with current utilization where the medication is more likely to be consumed prior to the expiration date.
  • Identified medications that are not likely to be consumed prior to their expiration date may be eligible for extended expiration dates.
